we recently purchased a Flagstaff HW and noticed the grey and black water are discharged separately.  Has anyone performed a mod to direct the grey water discharge into a "y" with the black discharge for camping with sewer hook ups.  Bigest difference is the discharge size.  Black water is 3" and grey is 1.5-1.75"...

I'd expect your grey to be 1.5".  With a little DIY you could easily add a dual waste gate like this so both grey and black waste out the same 3" bayonet outlet just as it does on any travel trailer.  I had one similar to this installed on my own travel trailer, replacing the original cable actuated system which never did work all that well.

We have the same setup with our Rockwood HW and I have included a coupler like the one shown in this link that has a seperate attachement for the grey water line to dump into the sewer. With hookups I have a grey water hose that feeds into the sewer from the black water tank, and when drycamping I use the same hose to feed into our 11 gallon tote, and do not have to fuss with the black water line.
